Best Choice Home Health Care provides daily and hourly services for the sick, elderly or disabled. Patients can pay out-of-pocket, through private insurances or through Medicaid/Medicare contract services established by Best Choice.


Medicaid/Medicare

Best Choice has contracts with Certified Home Health Agencies (CHHA), Managed Long-Term Care Plans and others to provide our services to persons with Medicaid and/or Medicare coverage. Under these contracts, a registered nurse will develop an individualized plan of care for each patient, and then Best Choice will provide a Home Health Aide or Personal Care Aide to the patient based on their geographic location, language spoken and care needs.


Private Pay/Private Health Insurance

Services can be paid out-of-pocket directly to Best Choice or through private insurance companies. Following an initial assessment by a registered nurse, a personal plan of care will be created based on the RN’s and/or physician’s recommendations. Patients will then be assigned a qualified Home Health Aide or Personal Care Aide based on patient location, language spoken and care needs. A follow-up visit with an RN will be scheduled as needed. All our services have flexible hours and can be coordinated within 48 hours of the request.

We accept credit cards as method of payment.


Requirements:

  • Patient must be a resident of the five boroughs of NYC, Nassau or Westchester County

  • Patient must be under the care of a physician

  • No age requirement

  • Patient must reside in a safe home environment

  • Relative or significant other who can be involved in patient's care

  • Sleeping accommodations must be available for 24-hour care aides

  • For private pay patients, an initial nursing assessment fee is required. In addition, a two (2) week deposit is necessary prior to start of services.
